How it works

The full system, in seven plain steps — from the first eligibility check to payday.

  1. 1. Check eligibility

It starts with Delaware Medicaid and a level-of-care assessment. Both need to be in place before paid care can begin.

  2. 2. Assessment & authorized hours

The health plan or the state sets the hours in a care plan. Those are the authorized hours. No authorized hours, no paid care.

  3. 3. Choose your caregiver

Your family chooses the caregiver: your adult child, a relative, a friend, or — for a child — a parent.

  4. 4. Caregiver onboarding

Before care begins, the caregiver completes onboarding: identification and work authorization, the Delaware Background Check Center criminal check and drug screening required by state law (16 Del. C. §§ 1145–1146), orientation covering the state’s required topics, and a competency test — passed before the first visit and repeated every year.

  5. 5. Care, verified

The caregiver clocks in and out in an EVV phone app. Under federal law, every visit records who provided the care, whom it was for, when it happened, and where.

  6. 6. We bill the plan

Spangler converts verified visits into claims to the member’s health plan. Families do not bill anyone.

  7. 7. Payday

The caregiver is paid regular W-2 wages by Spangler for the authorized, verified hours worked.
